NAME OF PROCEDURE: Laparoscopic converted to open appendectomy

DESCRIPTION OF TECHNIQUE/PROCEDURE:

A standard Hassan approach was made at the umbilicus.Access was obtained into

the abdominal cavity without difficulty.Pneumoperitoneum was established.

Visualization of the abdominal cavity saw no other pathology or injury upon

entry.Two more trocars were placed under direct visualization.


 

it was immediate apparent, there is multiple adhesions in the pelvis and right

lower quadrant.The tinea was followed down to the cecum and it was attempted

to remove the omentum from the bowel.Multiple attempts were made but the

omentum was too densely adhered.Any attempts to grass the tissue felt like

grafting wood.As the omentum could not be dissected free and everything was

so densely adhered, it was felt it was unsafe to proceed laparoscopically.


 

Incorporating two of the trocar sites, a midline incision was made.Access was

obtained without difficulty.Further exploration noted severely in density is

here phlegmon of omentum terminal ileum and appendix.Had to be dissected out

of the pelvic cavity.Further dissection of the terminal ileum off the

mesoappendix and appendix was performed.It was noted that the appendix had

perforated distally and approximately.This had been walled off by the bowel

and omentum.Eventually, the base of the appendix was identified and stapled

off.LigaSure was used to transect the mesoappendix. the area of the base of

the appendix was severely inflamed and friable.This was oversewn to reinforce

area.
